The Patient Navigator Registered Nurse is a professional nurse who
coordinates the delivery of nursing care for pre/postoperative Critical Limb
Ischemia patients through their hospital stay.
The Registered Nurse will provide support, education, referrals, coordination
of care and other assistance to Critical Limb Ischemia patients by working
cooperatively with ancillary and other patient team personnel in maintaining
standards of professional nursing practice in the clinical setting.
To provide one-on-one guidance to individuals as they move through the
health care continuum from pre-scheduling to day of procedure or surgery
and coordinate all discharge needs.
To eliminate any and all barriers to timely pre assessment testing for each
Critical Limb Ischemia case.
Provide a focused nursing assessment of peripheral vasculature and assist
in initiating orders as ordered by physician or by protocol.
To coordinate and participate in any research studies involving Critical Limb
Graduate of an accredited school of nursing.
Current state registration or eligibility to do so.
Current BLS for health care providers
ACLS- RNs with more than 1 year of experience should obtain certification
within 6 months of employment.
