The Licensed Practical Nurse is responsible for providing Nursing Care to residents in Long Term care, dementia unit, respite, hospice, rehabilitation, transitional care and their families/significant others. The Geriatrics & Extended Care unit offers an array of services ranging from: Pain management. Palliative Care. Geriatric Assessment Skills. Care of Dementia Patients. Physical Restraint Policy & Procedure. Discharge Planning and Teaching. Patient Wanderer Security System. Facilitate Inter-disciplinary Meetings. End of Life Care Planning. Rehabilitative Nursing. Interacting with other therapy modalities i.e. (RT, PT, OT, Restorative). Administering medications ordered by the provider in accordance with BCMA process . Completing treatments as ordered by the provider- works with wound care nurses as needed. Work Schedule: 03:45PM - 12:15AM WITH WEEKENDS AND HOLIDAY ROTATIONS. Financial Disclosure Report: Not required. PHYSICAL REQUIREMENTS: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
