This position is located within perioperative/OR, Nursing Service at the Phoenix VA Healthcare System (PVAHCS) in Phoenix, AZ. PVAHCS is a Level 1a major interdisciplinary teaching facility with functional service lines serving medical, surgical, mental health, and long-term care inpatients and outpatients with an Emergency Department and multiple community-based outpatient clinics (CBOC). The Registered Nurse (RN) in the ambulatory care setting is responsible for overseeing the daily operation of the designated clinic area. The RN demonstrates leadership in delivering efficient, comprehensive, and continuous patient-centric, holistic care through active collaboration, communication, and coordination of resources. The RN works in partnership with the team and panel of patients for effective acute and/or chronic care, disease management, and population health management. The RN uses the nursing process and evidence-based practice to collaborate with team members. The RN is responsible for collaborating with services internal and external to the VA to facilitate coordination and transitions of care to effectively meet the patients' needs. The RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's health goals and needs. The RN demonstrates leadership by serving as an advocate for patients. The RN provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses. The RN is responsible to participate in, and contribute to, the achievement of performance measures and institutional metric. The RN also provides nursing care and disease management to patients whether in clinic, primary care, and/or other specialist areas. The RN leads the nursing team, manages and develops the nursing clinic. As a clinical member of the management team, the RN is to be responsible for the safe, effective and timely delivery of care and future development of the overall service, working towards achievement of practice aims and objectives. The RN ensures that daily assignments and clinic responsibilities are made, prior to the clinic opening, based on current staffing and operational need. Assures that clinic staff respond appropriately to all drills relative to fire, safety, customer service, or emergencies. Ensures adequate stock levels of supplies and medications are maintained. Ensures proper disposal of regulated waste. Disseminates information to the interdisciplinary team. Serves as a resource for patient issues with interdisciplinary team members. Assists leadership with planning, organizing, implementing, & evaluating clinic in-service programs. Assists leadership with preparation for and achievement of Joint Commission and other regulatory and accreditation requirements. Maintains professional environment for clinical practice. Promotes a positive customer service image at all times. Reports daily attendance of nursing staff to management. Submits work orders for repair and notifies management of defective equipment. Ensures proper documentation (i.e., JSPR, post fall notes, etc.) and reporting of any significant events, to management, in a timely manner.
