Atlantic Health System is seeking a Hire Learning Residency Program, Atlantic Visiting at Morristown Medical Center, to participate in the Hire Learner Residency Program. In this position you will work work days and weekends closely alongside a preceptor with adult and geriatric patients of all backgrounds who require daily care in their homes. You are compassionate, thoughtful, and comfortable working with elderly and chronically ill patients. You will be responsible for managing a patient caseload; time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.
Essential Job Functions:
Perform thorough assessments that are systematic, comprehensive and accurate. Review charts and assessment and reports any abnormal findings to the Physician.
Utilize the appropriate scales in regards to fall prevention, pain management and cultural/educational needs of the patient.
Effectively assesse the potential discharge needs of the patient+/or family and acts upon these needs accordingly.
Consistently demonstrates competent clinical skills. Perform direct patient care activities in support of patient care delivery and collaborates with interdisciplinary team.
Provide competent and appropriate nursing care to assigned patients.
Demonstrate knowledge and skill of home-based equipment and procedures.
Demonstrate knowledge of medication education and administration.
Analyze situations critically and makes effective clinical decisions which reflect an integration of nursing knowledge and available resources (actual and potential problems).
Coordinate, evaluate and complete patient/family teaching. Identifies and reports all risk management issues in a timely manner.
Maintain complete and accurate documentation.
Demonstrate knowledge of documentation guidelines.
Assume primary responsibility for documenting components of the nursing process and patient's progress toward meeting outcomes in the patient's chart including patient education. Accurate documentation of any changes in the patient's status and interventions taken.
Accurately chart interventions and updates patients care plan each visit.
Review and update patient's medication profile each visit.
Demonstrate knowledge of policies and procedures and is able to identify areas for clinical and professional growth.
Attend pertinent educational offerings to keep abreast of agency and industry changes.
Actively pursue professional growth and development by taking the initiative to obtain information on new medications, procedures, equipment and technology.
Regularly attends minimum of 75% of team meetings or reads the minutes.
Keep current with team or agency functions.
Job Function: 05. Patient Satisfaction. Consistently responds to patient's/families and co-workers in a professional, courteous, positive manner to promotes an environment where the patient and family is at the center and the patient care team works cooperatively toward their goal and objectives. Maintains an open line of communication with patients and their families to expedite delays or concerns and notifies charge nurse about unresolved problems needing resolution. Maintains confidentiality according to HIPPA guidelines. Fosters PRIDE values. Collaborates with co-workers, Nurse Educator, Assistant Manager and Manager to reach target goal for patient satisfaction.
Job Function: 06. Safety. Consistently follows agency policy, regulatory guidelines and TJC regulations including hand hygiene and identifying patient's utilizing two patient identifiers. Demonstrates use of safe transfer techniques and proper body mechanics for self, and patient's. Follows appropriate infection control guidelines and utilizes appropriate PPE. Recognizes emergencies and reacts appropriately. Maintains a safe patient care environment including adhering to infection prevention and consistent implementation of hand hygiene. Demonstrates active involvement and support of all patient safety initiatives.

Graduate of an accredited Nursing Program required
Valid New Jersey Nursing License required
Current BLS certification required
1 - 2+ of previous experience of home care or acute care strongly preferred
