Job Summary:
Assesses and implements the care plans for assisted living Activities of Daily Living (ADL) of residents within the assisted living and memory care community -- with the supervision of the Wellness Nurse. The LPN is also responsible for ensuring continuity of care of the residents as well as supervising the care given by CNAs and supportive staff members. They will also participate in resident and family teaching. The LPN will function as a resident advocate with a focus on the community's mission.

DUTIES AND RESPONSIBILITIES:
Assists Wellness/Assessment Nurse in assessing residents on move-in as well as when conditions change. Documents changes in residents' condition and care on progress notes and care plan.
Assesses the resident's condition and needs, recommending appropriate action.
Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident, family and CNAs regarding pain management.
Administers medications and follows the five (5) medication rights and reduces the potential for medication errors.
Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
Meets current documentation standards and policies.
Notifies physician of changes in resident's condition and follows through until appropriate action is taken.
Notes and carries out physician's orders in a timely manner.
Gives a thorough report to oncoming shift by participating in walking rounds and documenting appropriately on calendar for upcoming needs.
Follows through as needed on information given by shift report, resident or family regarding resident concerns.
Treats residents, their families, visitors and co-workers with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
Demonstrates competence in technical and manual skills according to scope of practice for LPNs.
Functions as a resident advocate by protecting the resident's rights.
Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
Interacts with residents, families, co-workers, managers and other departmental personnel in a kind, knowledgeable and friendly manner.
Consistently involves resident, family and CNAs in planning for care, treatment, teaching needs and discharge plans.
Initiates and updates plan of care, as appropriate.
Participates on resident care conference and interdisciplinary team meeting as needed.
Functions as a leader for the CNAs and support staff; delegates in a manner that contributes to the excellence of the staff to which LPN is assigned.
Participates in orientation of new personnel and assists in performance evaluations when needed.
Follows proper infection control procedures and supervises supportive staff in infection control procedures.
Demonstrates ability to remain calm in emergency and/or stressful situations and gives everyone a feeling of confidence that situation is under control.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.
Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Consistently follows the policies and procedures of the facility.
Exhibits genuine interest in and appreciation of adults in later years of life and disabled.
Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.
Professional Requirements:
Adheres to dress code.
Completes annual educational requirements.
Maintains regulatory requirements.
Wears identification while on duty.

Attends department staff meetings as required within the department.
Reports to work on time and as scheduled; completes work in designated time.

Coordinates efforts in meeting Joint Commission, federal, state and local regulations and standards

Education/Experience Requirements:
Graduate of an accredited LPN program.
Current Licensed Practical Nurse in State of Montana.
Current BCLS certification, health care provider. Maintained per Benefis policy.
