Position Summary: The Ambulatory RN Ortho Care Manager provides patient care, triage, navigation, and education to patients receiving surgical and non-surgical care within the locations of the orthopedics clinic, and hospitals/ASC settings. This position will work in collaboration with clinic and inpatient teams to ensure quality outcomes and best practice implementation from initial contact through recovery. In partnership with providers, clinic staff, inpatient teams and community resources this role will assess and facilitate pre-operative and post-operative coordination of care. Additionally, this role provides education and needs assessments to prepare the post-operative patient for transition to the anticipated post-hospital care setting and guides the patient progress toward achieving clinical, quality, and cost-effective outcomes. The ambulatory RN ortho care manager participates in the development and evaluation of clinical guidelines and standards of care as well as assisting in; collection and analysis of aggregate data for an identified population of patients to drive process improvement and quality management efforts.
Minimum Requirements: 3 years of recent clinical hospital/ambulatory experience Healthcare financial and reimbursement knowledge preferred Leadership and emergency services experience a plus Above average clinical skills
Education/Licenses/Certifications: BSN or Associates Degree in Nursing with other Bachelors degree RN, licensed in the State of Washington
