Yuma Regional Medical Center (YRMC) Surgical Services is comprised of 12 operating rooms (OR's). Two ORs belong to Women's Surgical Services (WSS) and the remainder are housed in the Main OR (MOR). In the MOR, there is a designated room utilized as an Endoscopy suite and another as a Cystoscopy room. There are 9 PACU bays in the MOR and 4 PACU bays in WSS. The PACU department provides staffing to both the MOR and WSS, as well as recovers NORAS, pediatric endoscopy procedures, ERCP's, EUS's and the inpatient endoscopy patient population. Patients who undergo cardiac catheterization are recovered by the Cath Lab team within a separate unit.
YRMC offers a vast number of surgical procedures with the exception of brain and neurology specialties. Patients who have undergone a cardiac procedure are transferred directly from the OR to the ICU for recovery, bypassing PACU. YRMC does not have a PICU, therefore, pediatric patients who require intensive care will require transfer to facility in Phoenix, Arizona.
The amount of orientation for the PACU position is dependent upon experience and skill level. The length of orientation will be determined by the PeriAnesthesia Manager and Educator. Orientation may take place on varying shifts outside of the shift hired for. Once off orientation, staff will be scheduled according to the hours in which they were hired. Due to the nature of surgery, there may be a need to flex hours either earlier or later, dependent upon the need of the department. In the event that this occurs, staff would be notified of such a need prior to their scheduled shift.
PACU RN core positions consist of mandatory scheduled call. The amount of call required varies each month due to variances in staffing numbers. These numbers are impacted by position vacancies, PTO, leave, and orientations. There is an average of 4 weekend and 4-weekday call shifts each month, again, varying more or less due to the reasons previously stated. Staff self-schedules call shifts based on a rotational basis. The person at the top of the list gets first pick of the call shifts, but in turn has more call shifts. The person at the bottom of the list may have less call shifts, however the options are limited. Each month, the list continues to rotate and the staff members move from top to bottom. Weekday call is from 1900-0730, unless it is a scheduled workday, then call begins once the scheduled shift is complete. Weekend call is split into two 12 hour shifts, 0700- 1900 and 1900- 0700, on Saturday and Sunday. Weekend call is utilized to supplement the two scheduled working staff nurses based on patient load and departmental demands.
While on call, the PACU nurse will also have a backup nurse available to assist, as well as the ability to pull the WSS call PACU RN for higher acuity patients,if needed.
YRMC PACU staffing and ratios follow ASPAN standards. Ratios are reflective of patient acuity; Phase I will remain at a 1:2 nurse patient ratio. ICU holds or patients who have a change in status requiring a higher level of care will require 1:1 nurse to patient ratio with additional resources available as needed. Phase II/extended care patients result in a 1:3- 5 nurse to patient ratio.

Position Summary: The competent RN has mastered basic technical and organizational nursing skills within an inpatient setting. He or she is skilled in evaluating patient responses and uses past experiences in providing care. Patients and families are viewed holistically, there is a focus on outcomes, and care is delivered using a systematic approach. The competent RN is able to make decisions guided by limited experience, as well as policies, procedures, guidelines and standards. He or she consults with other members of the healthcare team when necessary. At the competent level, the RN can comfortably care for patients in his or her clinical area.
Education Required: Arizona or compact state RN License; BCLS; Advanced Life Support Certification(s) as appropriate & required for assigned department.
Experience Required: Computer proficiency (must be able to pass computer proficiency assessment)
Experience Preferred: Experience with EPIC Electronic Health Record or similar electronic record systems.
