Physician Assistant Palliative Care
St. Joseph Heritage Medical Group is hiring a Physician Assistant/Nurse Practitioner to join a care team serving homebound patients throughout Sonoma County. You will be providing home-based care primarily to homebound patients, while also providing care for Palliative Patients. This is a 32-hour per week position with flexibility required during the week. Ideal candidates will have experience in Palliative Care, Hospice, Pain Management, or Family Medicine, but new graduates with relevant experience are welcome to apply.
