Registered Nurse - Home Based Primary Care
The Home Based Primary Care Registered Nurse (HBPC) provides home-based primary care to geriatric patients, identifying problems and areas of concern, developing and initiating a plan of action, documenting all findings and recommendations, and evaluating the effectiveness of action, as well as assure it is timely, of high quality and cost effective. The nurse will confer with other members of the HBPC team, as well as other healthcare professionals in planning and initiating appropriate care.
